Patient Experience & Satisfaction Surveys

If you have the time and wouldn’t mind, we’d love it if you shared your experience with us. We appreciate any feedback you feel comfortable sharing.

Phone Based Surveys (CrossRoads Group)

You may receive a follow-up phone call from The CrossRoads Group (360-512-3787 or 360-512-3969) asking about your recent experiences with us. CrossRoads surveys are HIPAA compliant and confidential. Your feedback is shared with our staff within 1 week of you completing your survey.

Text & Emails (SocialClimb)

You may receive a follow-up text or email with your provider’s image shortly after your most recent visit. The communication will ask you to complete a short, 5-question survey, and provide feedback on how your experience went. You can choose whether your review is private or on a social media site like Google Business or Facebook.

Upstream USA Surveys
Valley View Health Center is partnering with a non-profit organization called Upstream USA that is providing training on contraceptive counseling and care. Valley View Health Center would like to learn about our patients’ experiences with contraceptive counseling and care. You may be invited by text message or email to take a survey on this topic. Upstream USA is conducting this survey. It is anonymous and voluntary. The survey is available in many languages and takes 5-10 minutes to complete. Patients who complete the survey will receive a $10 e-gift card. This survey will be used to help improve health care services. Opting Out

We know you are busy! If at any time you wish to opt out of receiving surveys, please let a receptionist at our clinic know that you wish to opt out of phone, text, or email reminders and surveys. If you receive a phone survey, you can simply ask to be removed from the calling list. If by text or email simple respond STOP to survey requests to ask to be removed. Thank you!

Immediate Concerns

If you have a complaint that needs immediate attention, please contact your clinic, and ask to speak with the Regional Administrator. If you would like to send us a message through our website, click here to send us a message. The web based message goes to our administrative staff and will be followed up within 3 business days.
