Patient Experience & Satisfaction Surveys

If you have the time and wouldn’t mind, we’d love it if you shared your experience with us. We appreciate any feedback you feel comfortable sharing.

Phone Based Surveys (CrossRoads Group)

You may receive a follow-up phone call from The CrossRoads Group (360-512-3787 or 360-512-3969) asking about your recent experiences with us. CrossRoads surveys are HIPAA compliant and confidential. Your feedback is shared with our staff within 1 week of you completing your survey.

Text & Emails (SocialClimb)

You may receive a follow-up text or email shortly after your most recent visit with your provider’s image. The communication will ask you to complete a 5-question survey and provide feedback on your experience. You can choose whether your review is private or on a social media site like Google Business or Facebook.

Consumer Assessment of Health Care Providers and Systems (CAHPS) Surveys (CAHPS)

These surveys do not come from Valley View but from your insurance plan. This is one way insurance plans recognize providers and health organizations. Please complete these surveys even if you have already completed a survey with Valley View Health Center.

Opting Out of Valley View Surveys

We know you are busy! If at any time you wish to opt out of receiving surveys, please let a receptionist at our clinic know that you wish to opt out of phone, text, or email reminders and surveys. If you receive a phone survey, you can simply ask to be removed from the calling list. If by text or email simple respond STOP to survey requests to ask to be removed. Thank you!

Privacy

The agencies we request your feedback through are HIPAA compliant and never share your personal information beyond Valley View Health Center. Additionally, the only information they are provided is the minimum amount of information needed to request your feedback in a timely manner. They do not have access to your chart and will not add you to any other mailing lists.

If you are having difficulty with the cost of your care, please ask about our sliding fee discount scale. No one will be denied access to services due to inability to pay; and there is a discounted/sliding fee schedule available based on family size and income.
