New 340B Prescription Drug Rebate Pilot

We are writing to share information about an upcoming federal program that may affect how certain prescription medications are billed and managed beginning January 1, 2026. This program is called the 340B Rebate Model Pilot.

What is the 340B Program?

The 340B Drug Pricing Program is a federal program that helps health centers use limited resources to support patient care by allowing us to obtain certain medications at reduced costs.

What is changing under the pilot?

Under the new 340B Rebate Model Pilot, participating providers will no longer receive the drug discount upfront for certain medications. Instead:

  • Prescription drugs may be billed to insurance at the standard price.
  • Drug manufacturers will later provide a rebate to the provider.
  • These changes happen behind the scenes and are part of a national pilot program.

How could this affect you?

For most insured patients, there should be little or no change in how you receive care or medications. Your insurance coverage, pharmacy, and provider will remain the same, and your copay or coinsurance will continue to be determined by your insurance plan.

However, because this rebate model is new and there are uncertainties during the pilot period, your provider or pharmacist may talk with you about switching to a lower-cost alternative medication if one is available and clinically appropriate. Any medication change would be discussed with you and made only if it is safe and effective for your condition.

The federal government has selected the following 10 medications to participate in the 2026 340B Rebate Model Pilot:

  • Eliquis
  • Enbrel
  • Entresto
  • Farxiga
  • Imbruvica
  • Januvia
  • Jardiance
  • Novolog / Fiasp (insulin products)
  • Stelara
  • Xarelto

What do you need to do?

You do not need to take any action at this time. If your medication is included in the pilot and a lower-cost alternative may be appropriate, your care team or pharmacist will discuss options with you.
